The next steps for your participation in the program include:

• Complete the feedback survey as part of your registration for the final workshop.
Payment for using the decision aid tool with your patients will be issued upon completion of the survey.

• Attend a 60-minute virtual final workshop.
You will receive compensation of $1,000 for attending the final workshop.
